TRANSITION OF CARE COORDINATOR – 12482

QUALIFICATIONS:

Education: Bachelors Degree in Nursing required

License/Certifications: Current NYS Registered Nurse License; Certification in
Case Management (preferred)

Experience: Experience in an acute hospital setting with an understanding of
patient flow, discharge planning, care coordination, and hospital operations.
Experience with the EPIC preferred. Experience in Case Management, Care
Coordination, Utilization Management or Patient Flow strongly encouraged.

SUMMARY: The Transitions of Care and Patient Flow Coordinator serves as a
liaison responsible for facilitating efficient patient movement throughout the
hospital and supports timely, safe, and coordinated patient discharges. This
position will work as a collaborative partner to Case Management, Nursing,
Physicians/ Providers, Ancillary Services, and other members of the
interdisciplinary team to identify and resolve barriers to patient flow and
discharges. This role will help align teams, communicate priorities, escalate
barriers, and promote timely progression of care.

Salary Range: $27.81 – $48.94
