BEHAVIORAL HEALTH SPECIALIST – 12306

QUALIFICATIONS:

Education: Graduate from an accredited master s degree program in mental health

field.

License/Certifications: Provisional LMHC, LMSW, LCSW or higher. Certification
in
Case Management Preferred.


Experience: Previous experience in case management preferred. General computer
knowledge and capability to use computers required. Critical thinking skills
and ability to analyze complex data sets required.


Responsible to: Director of Outpatient Psychiatric Services

Responsible for: Center Medical Home Care Management


SUMMARY: The behavioral health specialist is a core member of the collaborative
care team, including the patient s medical provider and psychiatric consultant,
as well as the larger primary care team or medical team. The behavioral health
specialist is responsible for supporting and coordinating the mental and
physical health care of patients on an assigned patient caseload with the
patient s medical provider and, when appropriate, other mental health
providers.

TRAINING & LICENSURE REQUIREMENTS: It is preferred that the behavioral health
specialist have evidence of required training, however training will be
provided for those candidates that meet all other clinical licensure
requirements, so as to be able to provide brief psychosocial interventions
appropriate for primary care settings such as problem-solving treatment.

Salary Range: $24.72 – $39.01
