B.S in Pharmacy.
Pharm. D. preferred.

License/Certifications:

Current, active license to practice pharmacy in New York State
Board certification preferred.
Practice Eligibility Criteria:
o Completion of three (3) years of practice experience with emphasis in
infectious disease pharmacy activities.

Experience:

3 years hospital pharmacy experience.
Specialty Residency in Infectious Disease, advanced training, or equivalent
practical experience preferred.
Pharmacy Board Certification, preferred.
Must possess excellent interpersonal skills, verbal and written communication
skills as well as experience with clinical program development and/or clinical
management skills.

SUMMARY: This position is accountable for the provision of leadership and
oversight of antimicrobial stewardship. This position is responsible for the
coordination of various clinical programs and initiative related to
antimicrobial therapy to optimize anti-infective drug use and therapeutic
outcomes, reduce antimicrobial resistance, and reduce costs associated with the
prevention and treatment of infectious disease. This position will ensure the
development and implementation of mechanisms that promote medication and
patient safety and quality related to antimicrobial use at Columbia Memorial
Hospital.



Salary Range: $51.60 – $79.31
