Qualifications:

Education: Graduate from an accredited School of Nursing with RN or LPN
credentials.
Demonstrates an understanding of the NYS Practice Act.

License/Certifications: Licensed Practical Nurse or higher, licensed to
practice in NYS.

Experience: Previous experience in Care Coordination preferred; minimum of
three (3) years experience as a licensed registered or practical nurse,
preferably five (5) years. General computer knowledge and capability to use
computers required.
Critical thinking skills and ability to analyze complex data sets required.

Summary: The Care Coordinator works in collaboration with physicians,
patients/families and payers to promote healthcare services that include
support, education, efficiency, timeliness, and effectiveness while keeping in
mind the principles of Patient Centered Medical Home. They are continuously
working within the mission statement of Columbia Memorial Health, working
within the scope of practice as defined by licensure, and providing coordinated
care under the direction and supervision of the Medical Director of the Care
Centers and the designated Provider.

Salary Range: $22.00 – $33.00
